If you lost or misplaced your certificate of completion for any of our classes, you may request a copy/duplicate here.
IMPORTANT: We only have records from the past 5 years. If you took a class with us more than 5 years ago, we will not have any records, so please DO NOT submit a request here. In addition, we only have records for our agency, A+ Alcohol & Drug Offender Education Center. If you took your class with another agency you will need to contact them directly. If you are unsure of the time frame and or agency, please verify we have your records by emailing us at offendereducation@yahoo.com before submitting this request.
Certificate copy/duplicate fee is $25.00
Once your request is received, your certificate copy/duplicate will be emailed to you within 24-48 hours. If you do not receive your certificate copy/duplicate within this time frame, please be sure to check your spam/junk folder.
